Patients who are new to Dr. Bartlett’s cosmetic and reconstructive surgery practice will be asked to provide information about themselves in order that we may provide the safest and most effective care possible. Please take a moment to download and fill out a New Patient Registration Form and Adult or Pediatric Medical History Form. Candidates for Breast Reduction Surgery also should download and fill out the appropriate form found below that is required for insurance purposes.
